Isolate the failure
Record whether the fuse was placed, carried or merely left loose. Check the original spawn area after using the normal save flow; do not assume the object remains where it was dropped.

If a placed fuse vanishes or a loose one does not return, preserve the save and exact sequence. The patch does not publish a universal location map or promise a reset for every state.
